Family Medical Health Care

Dental Care
At the CAPWN Dental Clinic our goal is to ensure each patient receives quality, affordable dental care in a personal and caring manner. We believe in preventative care to ensure a lifetime of dental health.
Outreach & Enrollment
Community Action Health Center has Certified Application Counselors (CACs) and Navigators who are trained and educated in the different options for healthcare insurance and other types of Nebraska state assistance. CACs and Navigators provide year-round assistance with your Health Insurance Marketplace applications and/or state assistance programs like Medicaid, SNAP (food stamps) and ADC ( Aid to Dependent Children). Our services are free to the community. You do not have to be a patient of Community Action Health Center to be seen.

Diabetes Management
Self-management education and support—teaching about disease process, how to check blood sugar, how to use insulin if needed. Information about food choices and healthy lifestyle, and helping with whatever tools they need to self-manage their diabetes.

Migrant Health
Migrant Health provides health care for migrant and seasonal farm workers in 19 western Nebraska counties.
Services include medical services for all ages, x-ray, laboratory, pediatric care, geriatric care, wellness and health education.

Immunizations
Vaccinations prevent diseases that could be potentially contagious and dangerous to the health and welfare of our population.
Our Immunization Clinic offers children and adult immunizations at a reduced cost and allergy injections for both children and adults, under the authority of the Wyoming Allergy Clinic.
We are a COVID Vaccine Provider, offering all FDA-approved COVID-19 vaccines as recommended by the CDC free of charge. Walk-ins are welcome.

We are excited to announce an expansion of our team of expert mental health, medication management, and addiction providers.
We welcome previous Panhandle Health Group consumers in receiving their care at Community Action Health Center.

Reproductive Health Programs
At Community Action Health Center, we strive to provide competent, confidential and safe reproductive health care and counseling to male and female, adolescent through senior clients with the knowledge to make healthy lifestyle choices.

Educational Services
We provide educational opportunities for our clients and community on the following topics:
Abstinence, Basic Male & Female Anatomy and Physiology, Health Promotion with encouragement of Family Participation, Life Plan Assessment & Goals, Family Planning & Contraceptive Methods, Healthy Relationships, STD/HIV Risk Prevention & Education, & Infertility

Medical Services
We provide the following medical services in our reproductive health program:
Pap Smears, Pelvic Exam, Rectal Exam, Abnormal problem exams, Pregnancy Testing, STD treatment, Counseling & Referral

Screening Services
We provide the following screening services in our reproductive health program:
PID, UTI, STD’S/HIV, HIV Referrals for Treatment (Ryan White), Physical Exams, Cervical Cancer Screening, Anemia & Diabetic Screenings, & Hypertension Screenings
Educational Services
We provide referrals for Mammograms and Reproductive Cancers.
Immunizations
We provide immunizations for ages 19-26.
Other Services
Family Planning and Contraceptive Methods, Wellness, Condoms, Pregnancy Testing, Dispensing of Birth Control Methods, Confidential Services for Teens

Insurance
As a courtesy, CAPWN Health Center will gladly submit your claim to any private insurance company and Medicare, along with Medicaid from Nebraska and Wyoming for you. The patient’s insurance card must besubmitted at the time of service.
Sliding Discount
CAPWN offers a Sliding Fee discount to patients based on household size and income guidelines.
Payments
Payments are expected at the time of service. We accept cash, checks and credit cards.
